How the forms work

No form on this website sends your information to a server. When you submit a form, the website writes a message with what you have entered, then offers to send it by WhatsApp or by e-mail. Until you have sent this message, nobody receives it.

On a phone, when you attach documents, your device’s share function sends the message and the files together. On a computer, files cannot be attached automatically: the website tells you which documents to add to the conversation.

The website does not store any of the documents you select.

The website’s forms

• Home page and Contact page: contact and care request.
• Second medical opinion: medical question, specialty concerned and useful documents.
• Partners: cooperation request, with the professional details of your organisation.

Health information

Some requests involve sending information about your health. This information deserves particular care.

• Only send the documents relevant to your request.
• Exchanges currently take place by WhatsApp or e-mail, which are not channels specialised for health data.
• No secure upload portal is in service at present.

Consent

Forms with a consent box ask you to agree that your information will be used to respond to your request. You can withdraw this consent at any time by writing to info@polycrabatsevamed.com.

Planned changes

Receiving requests through a server, with an acknowledgement of receipt and controlled retention, is under consideration. The corresponding arrangements will be published on this page on the day they actually come into service.
